1. ASSUMPTION OF RISK & RELEASE OF LIABILITY

I, the undersigned participant or coach, acknowledge that boxing, sparring, and combat sports involve inherent risks of injury, including but not limited to bruises, sprains, fractures, concussions, or other serious bodily harm. I fully assume all risks associated with my participation in the Fight4Dallas Sparring Showcase at Triple D Performance Center and voluntarily choose to engage in this event.

I hereby release, discharge, and agree not to hold liable Fight4Dallas, Triple D Performance Center, event organizers, coaches, trainers, staff, and any affiliated individuals or entities from any claims, demands, damages, injuries, or losses incurred as a result of my participation in this event. This includes, but is not limited to, negligence, accidental injury, or unforeseen circumstances during training, sparring, or other activities within the venue.

2. CONSENT TO MEDICAL TREATMENT

In the event of an emergency, I authorize Fight4Dallas and its representatives to secure necessary medical treatment, including emergency transport and care. I understand that I am responsible for all medical expenses incurred due to participation.

3. FILMING & MEDIA RELEASE

I acknowledge that Fight4Dallas may record, photograph, or live stream portions of the event for promotional and commercial purposes. I hereby grant Fight4Dallas and its affiliates the irrevocable right to use my name, image, likeness, voice, and performance in any media format (including, but not limited to, social media, YouTube, promotional videos, and advertisements) without compensation.

I understand that I may not record, distribute, or publish any footage from this event without express permission from Fight4Dallas. Unauthorized filming or live streaming is strictly prohibited.

4. REGISTRATION, COMMUNICATIONS & DATA PRIVACY

By providing my personal information, I agree to receive event-related emails, text messages, and notifications (A2P Messaging) from Fight4Dallas, including updates, promotions, and future event invitations. Fight4Dallas will not share or sell personal data to third parties.

I understand that I may opt out of communications at any time by replying "STOP" to text messages or using the unsubscribe link in emails.

5. AGREEMENT & SIGNATURE

I acknowledge that I have read, understood, and voluntarily agree to the terms and conditions outlined above. I affirm that I am at least 18 years old, or if under 18, that my legal guardian will sign on my behalf.
